*In consideration of the permission granted to my child to participate in the class or classes, I hereby release Cirque Dance Studio and its agents, employees, and representatives from all action, claims, cause of action, damages or demands which my child or I, my heirs, personal representatives or assignees may have against Cirque Dance Studio for all unintentional or accidental personal injuries which my child may incur by participation in the above described activity.

I am aware that the activity my child will be participating in is strenuous and hazardous.  With full knowledge of the activity and danger involved, I accept and assume the risk for any unintentional or accidental injuries sustained by my child above named.  

I have read the entirety of this enrollment form and RELEASE OF LIABILITY and fully understand its contents.  I execute it voluntarily and with full knowledge of its significance.
